Job summary

Legacy Lifecare VNA/Hospice is actively seeking a Registered Nurse Case Manager (RN) in Massachusetts. The role is full-time, 40 hours per week, with a schedule of Monday–Friday, 8:00 AM–5:00 PM and occasional on-call rotations, offering $41.00–$43.00 per hour based on experience.

Responsibilities include in-home and assisted-living care, monitoring patient changes, basic clinical tasks, and supporting palliative/hospice care with pain and symptom management.

Qualifications

• MA RN license required.
• Minimum 1 year RN experience in home-care, hospice, rehab, or long-term care preferred.
• Reliable transportation and ability to travel to patient homes or facilities.
• Must be fluent in English, both written and spoken.

• Observe residents and chart/report changes such as response to medication or treatment.
• Perform basic resident care or treatments, including temperature and blood pressure monitoring.

• Ability to communicate fluently in English - both verbally and written.
