Job summary

Hospice Services of Massachusetts is seeking an experienced RN Case Manager to plan, organize, and direct hospice care for patients and families. This full-time, on-the-road position requires a valid MA RN license and CPR certification, plus 1–2 years of acute care and home care experience.

Compensation includes up to $51 per hour, weekly pay, mileage reimbursement, and benefits. Travel throughout Barnstable and Plymouth Counties is expected.

Qualifications

• Graduate of an accredited school of nursing.
• Must possess an active and unencumbered RN license in Massachusetts.
• 1–2 years of recent acute care experience in an institutional setting, and 1–2 years of recent experience in home care preferred.
• CPR certified.

Responsibilities

• Supervise hospice aides.
• Complete comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of the nursing process.
• Develop and implement individualized care plans.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Provide health care instructions to patients and families.
• Counsel patients and families on meeting nursing and related needs.

Education

We are seeking an experienced and compassionate RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families.

Up to $51 an hour, depending on experience, with weekly pay.

This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel to portions of Barnstable and Plymouth Counties.
