Job summary

Legacy Hospice, Inc. is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our mission-driven team. You will deliver high-quality end-of-life care to patients in their homes and support families with empathy and respect.

This full-time, salaried role requires an RN licensed in the employment state, with hospice/home health experience, and CPR/BLS certification. Pre-employment drug screening applies, including marijuana testing.

Qualifications

• RN licensed in the state of employment.
• BSN or CHPN preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.
• Active CPR/BLS certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients, coordinating care with the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management, medication, and end-of-life care.
• Monitor and update care plans based on ongoing patient assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in interdisciplinary team (IDT) meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
