Job summary

Ascension's St. Vincent PACE program in Indianapolis is seeking a full-time Day RN to provide direct nursing care to participants in the PACE setting. You will perform acute triage, blood draws, and wound care, and participate in the development, implementation, and evaluation of the interdisciplinary team (IDT) plan of care.

Some travel to participants' residences or care facilities may be required. Licensure as an RN and BLS within 30 days are required.

Qualifications

• BLS Provider required within 30 days (hire date or transfer date).
• Registered Nurse licensure required prior to hire date or transfer date.

Responsibilities

• Provide direct nursing care to participants in PACE according to policies and protocols.
• Perform acute triage, blood draws, and wound care; support after-hours/weekend on-call rotation.
• Participate in IDT plan of care development, implementation, and evaluation; update as needed.
• Attend IDT care plan meetings and participant/family meetings; may perform assessments in multiple settings including PACE Clinic or Adult Day Center.
