Job summary

Ascension is seeking an RN to orchestrate the full interdisciplinary plan of care from admission through discharge, ensuring a seamless patient experience. You will design personalized care strategies that integrate clinical requirements with patient and family values, and monitor progress with precise medical documentation.

Qualifications

• Licensed Registered Nurse in Florida prior to hire.
• BLS Provider certification preferred (AHA/ARC).

How you’ll make an impact in this role

• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
• Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date. Licensed as a Registered Nurse in Florida. Credentialed from State Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.

• Preferred Credential(s):
• BLS Provider. American Heart Association or American Red Cross accepted.

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
