Job summary

Ascension in Manhattan, KS is seeking a Registered Nurse to join the Case Management team on Day-Shift and PRN. You’ll coordinate care transitions, design discharge pathways, and advocate for patient resources to ensure safe, seamless progress after hospital care.

This role emphasizes collaboration across disciplines and ongoing evaluation of documentation for appropriate levels of care, with a focus on improving quality as patient needs evolve.

Qualifications

• Licensed RN in Kansas.
• BLS Provider preferred.
• American Heart Association or American Red Cross CPR accepted.
• Experience in case management is a plus.

Responsibilities

• Coordinate comprehensive care transitions by serving as the primary link between clinical disciplines to ensure seamless recovery.
• Design individualized discharge pathways that reflect patient values and family preferences for safe transitions from hospital to next care setting.
• Advocate for patient resources by removing barriers to care and ensuring medical necessity and support for diagnoses.
• Evaluate clinical documentation accuracy to reflect true complexity of care and recommended levels of care.
• Drive continuous care improvement by collaborating with the care transition team as health plans adapt to patient needs.

• Design individualized discharge pathways that integrate patient values and family preferences, ensuring a safe and sustainable transition from the hospital to the next stage of care.
• Advocate for vital patient resources by identifying and removing systemic barriers to care, ensuring every individual has the medical necessity and support required for their specific diagnosis.
• Evaluate clinical documentation accuracy to provide expert recommendations on levels of care, ensuring that medical records reflect the true complexity of the patient's journey and treatment options.
• Drive continuous care improvement through active collaboration with the care transition team, constantly re-evaluating health plans to adapt to a patient’s changing condition in real-time.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Licensed Registered Nurse credentialed from the Kansas Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
