Job summary

Elder Care Homecare is seeking a Registered Nurse Care Manager to join our growing team in Massachusetts. This role focuses on coordinating client care, supervising caregivers and nurses, and ensuring plans of care are implemented effectively in clients’ homes and during transitions from hospital or SNF settings.

The ideal candidate will be a licensed MA RN, comfortable training staff on-site, and available to work some weekends.

Qualifications

• Licensed MA RN with current license.
• Ability to work autonomously and manage client care.
• Willingness to cover weekends as needed.

Responsibilities

• Oversee client care plans and ensure quality outcomes.
• Build and maintain relationships with caregivers and nurses.
• Perform, update, and monitor the Plan of Care for each client.
• Conduct home visits and coordinate care in clients' homes.
• Monitor hospital discharges and transitions to home care.
• Complete administrative paperwork per policy and DOH requirements.

Key Responsibilities

• Overseeing Clientele that is assigned to your care
• Creating and maintaining relationships with our Caregivers and Nurses caring for your client
• Executing, implementing, and updating the Plan of Care for the client, ensuring the Caregiver is aware of changes
• Home visits - 75% of your time will be spent out in the field
• Monitoring Discharge from Hospital, SNF, or Rehab
• Completing administrative paperwork in accordance with our policy and DOH

Qualifications

• MA Registered Nurse with Up to Date License
• Ability to work autonomously
• Ability to work some weekends throughout the month
• Must feel comfortable clinically training RNs and Caregivers on-site to their respective duties for their clients
