Qualifications

• Excellent communication and relationship-building skills.

Responsibilities

• Oversees, coordinates, and provides care for assigned patients per Liberty Advantage model.
• Performs routine assessments and follows up to monitor patient changes.
• Completes telephonic assessments on new admissions within 24 hours and in person within 48 hours.
• Assists medical providers with discharge orders and related paperwork.
• Serves as primary contact for members, families, and providers.
• Educates patients and families about advance care planning and end-of-life decisions.
