Job summary

Liberty Advantage is seeking a Registered Nurse (RN license required) for a full-time role in Winston-Salem and surrounding areas. You will oversee patient care, conduct assessments, and collaborate with providers to ensure timely interventions and smooth transitions.

Ideal candidates have experience in adult, family, or geriatric settings and strong communication skills. Willingness to be on call and to educate patients and families is a plus.

Qualifications

• Must have a current, unrestricted Registered Nurse license in the state of assignment or the ability to obtain.
• Certification as a Registered Nurse by a national credentialing body.
• Clinical background in adult, family or geriatric setting preferred.
• Past experience working in long-term care, a nursing home or with seniors in other settings preferred.
• Excellent communication and relationship building skills preferred.
• Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook.

Responsibilities

• Oversees, coordinates, and provides care for assigned patients in the assigned facilities.
• Conducts routine assessments for assigned patients based on risk schedule.
• Completes acute assessments to evaluate patient changes and facilitates prompt medical provider response.
• Follows up to monitor member response to interventions and report progress to medical provider.
• Completes telephonic assessment on new admissions within 24 hours and in person within 48 hours.
• Assists medical provider in preparing discharge orders and paperwork for transfers or discharges.
• Serves as primary contact for members and families, coordinating with providers.
• Notifies family of changes in condition and updates to care plan.
