Liberty Health in Asheville, North Carolina, seeks an experienced Registered Nurse (RN license required) for the Liberty Advantage program. This full-time RN will oversee and coordinate patient care across facilities, conduct risk-based assessments, and support discharge planning while collaborating with medical teams and families.

The role emphasizes early problem identification, chronic disease management, and education for patients and staff.

Qualifications

• Must have a current, unrestricted Registered Nurse license in the state of assignment or ability to obtain.
• Clinical background in adult, family or geriatric care preferred.
• Experience in long-term care or senior care settings preferred.
• Excellent communication and relationship-building skills preferred.
• Proficiency in PC-based Word processing including Microsoft Word and Outlook.

Responsabilités

• Oversees, coordinates, and provides care for assigned patients in line with Liberty Advantage Model of Care.
• Conducts routine assessments for assigned patients per risk schedule.
• Completes acute assessments to evaluate patient changes and prompt provider response.
• Maintains communication with families and providers regarding patient progress.
• Assists with discharge planning, instructions, and paperwork for transfers.
