Behavioral Health Network (BHN) is seeking a Registered Nurse to join the PACT team. The role focuses on direct and indirect nursing services, coordinating physical-health assessments, and integrating medical and psychiatric care to support person-centered treatment.

The position requires a BS in a related field, an active RN license, and knowledge of MAP and care coordination. The role offers growth opportunities within BHN’s established Western Massachusetts program.

Qualifications

• Bachelor's degree in a related field required.
• Current, unrestricted RN license and required nursing certifications required.
• At least six months of experience in human services preferred.
• Knowledge of MAP requirements, medication management, physical-health assessment, treatment planning, care coordination, and community-based nursing practice.

Responsabilités

• Provide direct and indirect nursing services to individuals served by BHN's PACT team.
• Coordinate and complete assessments of physical-health needs and referrals to providers.
• Follow MAP policies and document health- and medication-related information.
• Manage medications, medical supplies, and related clinical workflows.
• Develop and document treatment plans, health goals, and recovery objectives.
