Job summary

Behavioral Health Network, Inc (BHN) is seeking a Registered Nurse or Licensed Practical Nurse to join the Program of Assertive Community Treatment (PACT) team in Western Massachusetts. The role involves coordinating medical and psychiatric care, managing medications, and supporting treatment goals within MAP-guided procedures.

Qualifications

• Bachelor’s Degree in related field preferred.
• Registered Nurse (RN) licensure or Licensed Practical Nurse (LPN) certification required.
• Must obtain MAP certification within introductory period of employment.
• Minimum 6 months in Human Service field preferred.
• Valid driver's license and reliable vehicle required for all working hours to transport individuals served.

Responsibilities

• Ensures all staff are following MAP Policies and Procedures and follows these procedures.
• Completes health and medication-related documents as indicated and in accordance with MAP regulations.
• Under the direction of the team Psychiatrist, Nurse Manager, and in collaboration with other RNs on the team, manages pharmaceuticals and medical supplies.
• Coordinates, schedules, and administers medical assessments of individuals’ physical health, with referrals as needed.
• Ensures change of shift log is completed each shift and handoffs occur smoothly to oncoming staff.
• Provides safe and timely transportation in own vehicle for medical appointments and activities as needed.
• Provides ongoing information, periodic reviews, and documentation to support Treatment Plan goals.
• Maintains OSHA guidelines.
• Provides coverage at all PACT locations as needed.
• Completes individually assigned program-specific responsibilities.

What You’ll Do

• Provides safe and timely transportation in own vehicle, as needed for medical appointments, recreational activities, shopping, errands, etc.
• Provides ongoing information, periodic reviews, and documentation to support treatment and Treatment Plan goals and objectives.
• Maintains OSHA guidelines.
• Provides coverage at all PACT locations as needed.
• Completes individually assigned program-specific responsibilities.

Who You Are

• Bachelor’s Degree in related field preferred.
• Registered Nurse (RN) licensure or Licensed Practical Nurse (LPN) certification required.
• Must obtain and maintain MAP certification within introductory period of employment.
• Minimum 6 months in Human Service field preferred.

Pay: $42 - $43 per hour
