Job summary

Ascension in Indianapolis, IN is seeking a Registered Nurse to manage bed placement and coordinate patient care in the Care Coordination Command Ctr. This full-time night shift role involves working with multiple units to ensure timely bed assignment and cohesive discharge planning.

Responsibilities

• Manage the bed board and assign beds for patients staying overnight.
• Coordinate with units and leadership for bed placement; non-bedside role.
• Implement and monitor patient care plans; document and communicate patient status.
• Serve as primary coordinator of multidisciplinary patient care.
• Record and execute physician and nursing orders.
• Assess and coordinate discharge planning with the healthcare team.

How you’ll make an impact in this role

• Work the bed board and assign the beds to all patients who stay overnight.
• Placement Nurses work with all units and their leadership, Non bedside role.
• Implement and monitor patient care plans. Monitor, record, and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.

What Minimum Requirements You’ll Need

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

What Additional Preferences We're Seeking

At least 2 years experience as a bedside RN

Prior ED, ICU, And/or Bed Placement Experience Preferred
