  • Location: Avamere Transitional Care at Sunnyside - 4515 Sunnyside Rd. SE, Salem, OR 97302

Responsibilities
  • Provide direct patient care, maintaining nursing standards.
  • Complete documentation during resident admissions, transfers, and discharges.
  • Coordinate with staff to plan shift services and activities.
  • Administer medications accurately and review medication records.
  • Review and update care plans based on residents’ needs.
  • Supervise CNAs and other support staff.
  • Accurately chart care provided and residents' responses.
  • Participate in facility surveys and maintain confidentiality per HIPAA guidelines.
Qualifications
  • Nursing degree from an accredited institution.
  • Current, active RN or LPN license in this state.
  • Knowledge of nursing practices, laws, and regulations applicable to nursing facilities.
  • Experience with Electronic Medical Records and documentation systems.
  • Strong communication, organization, and prioritization skills.
  • Customer service experience required.
  • Knowledge of Medicare and Medicaid reimbursement programs preferred.
