Lakeside Health & Rehab is seeking an RN MDS Coordinator to join our clinical leadership team! This is an excellent opportunity for an RN who understands the importance of accurate assessments, strong documentation, regulatory compliance, and interdisciplinary collaboration in a skilled nursing environment.
As our RN MDS Coordinator, you’ll play an essential role in connecting clinical care, documentation, quality outcomes, and reimbursement—all while helping ensure our residents receive the individualized care they deserve.
Complete accurate and timely MDS assessments in accordance with RAI and CMS guidelines
Coordinate the MDS process and ensure assessments are completed within required timeframes
Lead and participate in interdisciplinary care planning
Review clinical documentation to support accurate coding and PDPM reimbursement
Collaborate with nursing, therapy, social services, dietary, and other departments
Monitor quality measures and support facility quality improvement initiatives
Identify documentation opportunities and educate nursing staff on best practices
Ensure resident care plans accurately reflect individual needs, goals, and clinical conditions
Stay current on MDS, RAI, CMS, and regulatory requirements
Support survey readiness and ongoing regulatory compliance
Active Registered Nurse (RN) license in Virginia – REQUIRED
Previous MDS experience strongly preferred
Skilled nursing/long-term care experience preferred
Strong understanding of MDS, RAI guidelines, PDPM, and care planning
Knowledge of CMS requirements and skilled nursing documentation practices
Excellent attention to detail and organizational skills
Strong clinical assessment and critical-thinking abilities
Ability to effectively collaborate with clinical and interdisciplinary teams
MDS/RAI certification preferred or willingness to obtain
