Brevillier Village Nonprofit Senior Housing and Health Care in Erie, PA seeks a Registered Nurse Assessment Coordinator (RNAC) to lead resident assessments and care planning within the Nursing department.
You will coordinate MDS processes, complete Medicare/Medicaid paperwork, audit nursing documentation, train staff on documentation standards, and ensure compliance with evolving regulations. This is a full-time leadership role with varied hours and a focus on impactful resident care.
REQUIREMENTS: Possess current or Pending Pennsylvania RN license with 2-3 years’ experience in a long-term care facility.
Proficiency with computers with willingness to learn insurance programs.
Good clinical nursing skills and assessment skills.
Self-motivated and kept up to date with changing regulations.
Good employment/attendance record.
Must be able to stand, walk, sit, squat, bend, push/pull, lift a minimum of 25 lbs. alone or 50 lbs. with assistance.
Conduct chart audits for proper nursing documentation and provide nursing staff with proper training for the preferred quality and quantity of documentation required.
Coordinate and oversee the resident assessment (MDS) and care planning system to assure each resident is assessed on a timely basis and has a current care plan established.
Coordinate and complete the required paperwork for Medicare, Medicaid, Department of Health and other regulatory agencies as it relates to the residents and the medical and nursing services they receive.
Successful completion of training period required.
