Laurel Lakes Rehab and Wellness Center in Chambersburg, PA is seeking a full-time Registered Nurse Assessment Coordinator (RNAC) to lead our nursing team in comprehensive resident assessments and the RAI process. The RNAC collaborates with the interdisciplinary team to ensure compliance with state and federal regulations and payer requirements.

The ideal candidate holds an RN license, has long-term care assessment experience, and is proficient in PPS/PDPM, Medicare billing, and regulatory

Qualifications

• Registered Nurse with assessment skills in a long-term care setting.
• Knowledge of state and federal regulations.
• Current RN licensure.
• PA CMI knowledge and Medicare billing experience.

Responsibilities

• Conduct and coordinate resident assessment per state requirements and facility goals.
• Coordinate RAI process in line with Medicare, Medicaid, OBRA and payer rules.
• Develop, implement and maintain a quality assurance program for the resident assessment.
• Participate in facility surveys by government agencies.
• Monitor QI and QM reports to trigger corrective actions.
• Educate interdisciplinary team involved in the RAI process.

Laurel Lakes Rehab and Wellness Center is currently looking for a full time Registered Nurse Assessment Coordinator (RNAC) to assist with leading our nursing team! Are you a compassionate and enthusiastic Registered Nurse (RN) with assessment experience in long-term care? Then this might be the perfect opportunity for YOU!

Requirements of the Registered Nurse Assessment Coordinator (RNAC):

• Registered Nurse (RN) with assessment skills in a long-term care setting
• Must have knowledge of state and federal regulations
• Associate's in Nursing Required, BSN preferred.
• Must have at least 1 year experience as an RNAC in a skilled nursing facility completing MDS
• Proficient in PA CMI
• Registered Nurse Assessment Coordinator (RNAC) must have Medicare billing knowledge and experience.
• Knowledge of PPS (PDPM)
• Experienced with Managed Care and Insurance Reviews
• Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.

Responsibilities of the Registered Nurse Assessment Coordinator (RNAC):

• Conduct and coordinate the completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility.
• Conduct and coordinate the completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs.
• Develop, implement, and maintain an ongoing quality assurance program for the resident assessment.
• Participate in Facility surveys (inspections) made by authorized government agencies.
• Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.
• Provide initial and ongoing education with the interdisciplinary team involved in the RAI process.
