Job summary

Five Star Nursing is seeking a Registered Nurse Assessment Coordinator (RNAC) to lead the MDS/RAI processes, ensure accurate clinical documentation, and maintain regulatory compliance across resident assessments and care planning.

This role directly impacts care quality and reimbursement while coordinating with interdisciplinary teams to ensure survey readiness and ongoing improvement. A strong focus on education and regulatory updates supports staff development and regulatory excellence.

Qualifications

  • Active RN license in the state of employment.
  • At least 2 years of experience in skilled nursing, long-term care, rehabilitation, or senior care.
  • Minimum of 1 year of MDS/RNAC experience.
  • Strong knowledge of MDS 3.0, RAI processes, and CMS regulations.
  • Excellent organizational, assessment, and documentation skills.
  • Proficiency with Electronic Medical Records (EMR) systems.
  • Critical thinking skills and attention to detail.

Responsibilities

  • Coordinate and manage the MDS/RAI process in line with federal and state regulations, ensuring timely and accurate assessments.
  • Complete MDS assessments, CAAs, and care plans reflecting resident needs and regulatory compliance.
  • Ensure documentation supports coding for reimbursement through thorough record review.
  • Collaborate with interdisciplinary teams to develop and update individualized care plans.
  • Monitor quality metrics related to assessments and care plans, driving improvements.
  • Schedule and oversee assessment timelines to guarantee timely MDS submissions.
  • Participate in Medicare reviews, utilization meetings, and audits for survey readiness.
  • Educate and support nursing staff on documentation standards and regulatory updates.
  • Conduct resident assessments and observe condition changes to inform care adjustments.
  • Analyze outcomes and propose process improvements to enhance care quality.

Overview

As a Registered Nurse Assessment Coordinator, you will lead the Resident Assessment Instrument (RAI) and MDS processes, ensuring precise clinical documentation, regulatory compliance, and optimal resident outcomes. Key Responsibilities
  • Coordinate and manage the MDS/RAI process in line with federal and state regulations, ensuring timely and accurate assessments.
  • Complete MDS assessments, Care Area Assessments (CAAs), and care plans that accurately reflect resident needs and support regulatory compliance.
  • Ensure documentation supports appropriate coding for reimbursement through thorough review of resident records.
  • Collaborate with interdisciplinary teams to develop, review, and update individualized care plans focused on resident-centered care.
  • Monitor quality metrics related to assessments and care plans, implementing improvement initiatives as needed.
  • Schedule and oversee assessment timelines to guarantee timely submission of MDS data.
  • Participate in Medicare reviews, utilization meetings, and audits, supporting survey readiness.
