Five Star Nursing in Allentown, PA is seeking a Registered Nurse Assessment Coordinator (RNAC) to lead the MDS/RAI process, ensure regulatory compliance, and support high-quality resident care through precise documentation.

You will collaborate with interdisciplinary teams, educate staff on standards, and drive survey readiness while improving care plans and outcomes.

Qualifications

  • Active RN license in the state of employment.
  • At least 2 years of experience in skilled nursing, long-term care, rehabilitation, or senior care.
  • Minimum of 1 year of MDS/RNAC experience.
  • Strong knowledge of MDS 3.0, RAI processes, and CMS regulations.
  • Excellent organizational, assessment, and documentation skills.
  • Proficiency with EMR systems.
  • Critical thinking skills and attention to detail.

Responsibilities

  • Coordinate and manage the MDS/RAI process in line with federal and state regulations, ensuring timely and accurate assessments.
  • Complete MDS assessments, CAAs, and care plans that accurately reflect resident needs and support regulatory compliance.
  • Ensure documentation supports appropriate coding for reimbursement through thorough review of resident records.
  • Collaborate with interdisciplinary teams to develop, review, and update individualized care plans focused on resident-centered care.
  • Monitor quality metrics related to assessments and care plans, implementing improvement initiatives as needed.
  • Schedule and oversee assessment timelines to guarantee timely submission of MDS data.
  • Participate in Medicare reviews, utilization meetings, and audits, supporting survey readiness.
  • Educate and support nursing staff on documentation standards, regulatory updates, and assessment best practices.
  • Conduct resident assessments and observe changes in condition to inform care adjustments.
  • Analyze clinical outcomes and recommend process enhancements to improve overall care quality.
