Spanish Hills Wellness Suites in Las Vegas, NV seeks an RN Assessment Coordinator (RNAC, MDS) to oversee patient/resident care from admission to discharge. Salary range shown reflects hourly wages of $41.00–$46.00 with comprehensive benefits for full-time staff.

The role requires an active RN license, 2+ years of nursing experience, including long-term care, and familiarity with RAI/MDS processes and ICD-10 coding. Reliable transportation is required.

Qualifications

• Must have a current, unencumbered RN license in state of practice.
• Minimum of 2 years of direct hands-on nursing experience.
• Minimum of 2 years of long-term care experience.
• Preferably at least 1 year of RAI experience (focus on MDS).
• Proficiency in ICD-10 coding experience required.
• Reliable transportation required.

Responsibilities

• Coordinate patient/resident care from admission through discharge.
• Identify/monitor resident skilled services.
• Coordinate the Resident Assessment Instrument (RAI) process.
• Complete an accurate Minimum Data Set (MDS) and develop the interdisciplinary Plan of Care.
• Monitor status to identify changes and barriers to discharge.
• Participate in the daily Medicare PPS Interdisciplinary Team Meeting (ITM).
• Establish and maintain relationships with the healthcare team and nursing management.

Job description

The Registered Nurse Assessment Coordinator (RNAC, MDS), under the direction of the Director of Nursing (DON) is responsible for the coordination of patient/resident care from admission through discharge. USD $41.00 - USD $46.00 /Hr. The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.

• Identifying/monitoring resident skilled services
• Coordinating the Resident Assessment Instrument (RAI) process
• Completing an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care.
• Monitoring patient/resident status to identify changes in patient or resident condition, barriers to discharge, and progress toward care goals.
• Participating in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM).
• Establishing and maintaining positive relationships with all members of the healthcare team and will function as part of the nursing management team.
• Must have a current, unencumbered RN license in state of practice
