Key Responsibilities:
- Perform nursing assessments according to service line requirements with high clinical competence.
- Evaluate clients' physical health and needs for services.
- Complete screenings, record medical histories, vital signs, and coordinate action plans.
- Monitor health status during services.
- Coordinate care with the treatment team based on client needs.
- Create action plans addressing physical and mental health needs.
- Oversee infection control, hygiene, and related health issues under nursing supervision.
- Provide health education on disease prevention and risk reduction.
- Arrange medical appointments and manage emergencies.
- Consult with physicians or advocate for services as needed.
- Manage medication requirements, educate clients, and oversee medication administration.
- Review lab results, monitor medication side effects, and coordinate medication needs.
- Assist with client exams, labs, and special procedures.
- Complete documentation promptly and accurately.
- Engage with long-term and short-term organizational goals.
- Perform other duties as assigned.
Knowledge, Skills, and Abilities:
- Effective communication skills.
- Proficiency with computers and software applications.

- Effective communication with diverse populations.
- Ability to handle behavioral health crises professionally.
- Thorough nursing knowledge related to mental health and substance abuse.
- Understanding of relevant policies, procedures, and regulations.

- Ability to interpret medical documents and records.
- Effective written and verbal communication skills.
Qualifications:
- Current RN license in the practicing state; willingness to become licensed in additional states.
- FQHC: BCLS certification required; ACLS preferred.
Employment Requirements:
- Background checks (criminal, driving, abuse/neglect).
- Completion of New Hire Orientation and ongoing training.
- Maintenance of licenses and certifications.
- If transporting clients: valid driver’s license and proof of insurance.
Physical Requirements:
- Sedentary work involving lifting no more than 10 pounds and occasional walking or standing.
