Overview: Licensed Practical Nurse, Assertive Community Treatment Team

Heritage has full-time community-based LPN positions working with our Assertive Community Treatment (ACT) team, providing services to individuals with severe and persistent mental illness or dual disorders. The team is multidisciplinary, working with individuals in the community across various settings, including homes, hospitals, emergency rooms, and social service agencies. The goal is to maintain optimal wellness and address health needs as they arise.

Core Responsibilities Include:

  • Engage persons served in their care within community settings.
  • Identify key health indicators to monitor overall wellness and healthcare needs.
  • Assess and monitor vital signs such as blood pressure, blood sugar, and weight at established intervals.
  • Ensure referrals, linkage, and attendance for medical evaluations, specialty healthcare, and other health activities.
  • Collaborate and consult with healthcare providers to share information effectively.
  • Request and confirm receipt of evaluations, labs, tests, and healthcare follow-ups.
  • Communicate effectively with providers, staff, and persons served.
  • Participate in treatment team meetings to coordinate care and recovery efforts.
  • Provide individual and group interventions, including education, training, and support for behavioral health and health conditions.
  • Lead wellness activities such as walking, yoga, smoking cessation, diabetes management, etc.
  • Complete documentation and tracking promptly.
  • Perform other duties as assigned or negotiated with the supervisor.

Knowledge, Skills, and Abilities:

  • Maintain confidentiality per Illinois laws and regulations.
  • Adaptability to changing program needs and schedules.
  • Supportive and respectful interactions with individuals and staff.
  • Strong problem-solving and decision-making skills.

Education and Experience:

  • High school diploma or equivalent.
  • Active Illinois licensed Professional Nurse.
  • Experience in behavioral health or human services preferred.

Salary Range: $54,080-$72,800 annually ($26-$35 per hour), depending on education and experience.
