As our ACT nurse, you will work as a member of the Assertive Community Treatment (ACT) Team, serving as a critical component of care to adults with severe mental illness in their homes, places of employment, and communities. You will work with a team of multidisciplinary staff to help each client succeed in living happy, successful, and productive lives, while staying integrated with friends, family, and community.
In your role, you will meet with clients in their homes, community settings, and the office to provide medical treatment and support services such as vital signs, labs, administering long-acting injectables, medication management, and coordinating care with medical providers. This position involves client transportation using a company car, as needed, and a one-week on-call rotation every 7-10 weeks, which can be handled virtually or in person, earning additional PTO. The primary focus is on community-based client interactions, with some time spent in the office or via Zoom/phone.
Assertive Community Treatment (ACT) was developed in the 1970s and is a well-researched evidence-based practice for adults with severe mental illness. It offers person-centered assistance including housing, recovery, vocational support, and services from nurses, psychiatrists, and therapists, enabling clients to transition to lower levels of care or move towards independence, thereby improving their quality of life.
