Psychiatric Nurse - Assertive Community Treatment (ACT)

Summary of Work

Assess and monitor clients’ psychiatric, medical, and social needs. Oversee medication administration under the supervision of a psychiatrist or nurse practitioner. Collaborate with providers to coordinate treatment, and manage supplies.

This role functions as a team member of the Assertive Community Treatment Team, supporting individuals with chronic mental illness to reduce symptoms and improve daily functioning.

Compensation

Between $53,100 and $67,177 annually, based on experience.

Work Schedule

40 hours/week, Monday to Friday, 8:30 a.m. to 5:00 p.m.

Minimum Qualifications
  • Graduate of an accredited nursing program and licensed as an RN in Iowa.
  • Two years of experience, preferably in mental health or substance abuse settings, with knowledge of primary healthcare issues.
  • Valid driver’s license and acceptable driving record.
Essential Duties and Responsibilities
  • Develop, implement, and evaluate care plans for clients.
  • Act as case manager for clients on your caseload and support team members.
  • Ensure coordinated, best-practice care, including risk assessment and multi-agency collaboration.
  • Support clients and families during inpatient stays and community reintegration.
  • Conduct psychiatric assessments, including emergency evaluations.
  • Assess physical health, make referrals, and coordinate care.
  • Screen for health risks and connect clients with primary care providers.
  • Coordinate healthcare with IHH providers and implement wellness activities.
  • Provide crisis intervention when needed.
  • Manage medication administration with the team psychiatrist or nurse practitioner.
  • Maintain accurate clinical records and treatment plans.
  • Provide health education and training to staff.
  • Build community connections to support agency goals.
  • Perform other related duties as assigned.
