Essential Job Functions:
- Perform nursing assessments according to service line requirements, demonstrating high clinical competence.
- Evaluate clients' physical conditions and service needs.
- Complete screenings, record medical histories and vital signs, and coordinate action plans based on results.
- Monitor health status during services.
- Collaborate with the treatment team to coordinate care.
- Create action plans addressing physical and mental health treatment needs.
- Oversee infection control, hygiene, and related health issues under nursing leadership.
- Provide health education on disease prevention and risk reduction.
- Arrange medical appointments and manage emergencies like injuries or drug reactions.
- Consult with physicians or advocate for services through managed care organizations.
- Manage dietary needs for medical conditions.
- Review medications, educate clients and guardians, and oversee medication administration.
- Coordinate medication needs with pharmacies and monitor side effects.
- Assist with client exams, labs, and procedures as requested by physicians or advanced providers.
- Complete documentation promptly per policies.
- Engage with the company's long-term and short-term goals.
- Perform other duties as assigned.

- Competence in interpreting medical documents and writing reports.
Experience and Education:
- Current RN license in the practice state; willingness to become licensed in additional states and provide telehealth services.
- FQHC: BCLS certification required; ACLS preferred.
Other Requirements:
- No supervisory requirements.
- Background checks, orientation, and ongoing training are mandatory.
- Valid driver’s license and insurance verification if transporting clients.
Physical Requirements:
- Sedentary work involving occasional walking and standing, lifting up to 10 pounds.
