Qualifications

  • Must have a current RN license and be willing to obtain additional state licenses.
  • BCLS certification required; ACLS preferred.

Responsibilities

  • Perform nursing assessments and evaluate clients' physical conditions.
  • Collaborate with the treatment team to coordinate care and create action plans.

Essential Job Functions:
  1. Perform nursing assessments according to service line requirements, demonstrating high clinical competence.
  2. Evaluate clients' physical conditions and service needs.
  3. Complete screenings, record medical histories and vital signs, and coordinate action plans based on results.
  4. Monitor health status during services.
  5. Collaborate with the treatment team to coordinate care.
  6. Create action plans addressing physical and mental health treatment needs.
  7. Oversee infection control, hygiene, and related health issues under nursing leadership.
  8. Provide health education on disease prevention and risk reduction.
  9. Arrange medical appointments and manage emergencies like injuries or drug reactions.
  10. Consult with physicians or advocate for services through managed care organizations.
  11. Manage dietary needs for medical conditions.
  12. Review medications, educate clients and guardians, and oversee medication administration.
  13. Coordinate medication needs with pharmacies and monitor side effects.
  14. Assist with client exams, labs, and procedures as requested by physicians or advanced providers.
  15. Complete documentation promptly per policies.
  16. Engage with the company's long-term and short-term goals.
  17. Perform other duties as assigned.
Knowledge, Skills, and Abilities:

  • Competence in interpreting medical documents and writing reports.
Experience and Education:
  • Current RN license in the practice state; willingness to become licensed in additional states and provide telehealth services.
  • FQHC: BCLS certification required; ACLS preferred.
Other Requirements:
  • No supervisory requirements.
  • Background checks, orientation, and ongoing training are mandatory.
  • Valid driver’s license and insurance verification if transporting clients.
Physical Requirements:
  • Sedentary work involving occasional walking and standing, lifting up to 10 pounds.
