Job summary

Millennium Physician Group is seeking a Registered Nurse to join our Home Health Team in Sarasota County. You will provide in-home, patient-centered care with a focus on assessments, care planning, and education to improve patient outcomes.

The role involves direct nursing care, wound management, medication administration, and collaboration with the care team, while maintaining compliant documentation and regulatory standards.

Qualifications

• Current and valid Registered Nurse (RN) license in Florida.
• Minimum of 1 year of experience in a hospital or skilled nursing facility.
• Strong clinical skills and ability to educate patients and families.

Responsibilities

• Assess patients' physical, mental, and emotional health during home visits.
• Develop and implement individualized care plans and disease management.
• Educate patients and families on disease management, medications, and self-care.
• Provide direct patient care including wound care, medications, injections, and IV therapy.
• Collaborate with physicians, therapists, and other care team members for coordinated care.
• Maintain accurate and timely documentation of assessments and progress.
• Adhere to HIPAA and Medicare regulations.

What You’ll Do

• Patient Assessment: Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
• Care Planning: Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
• Patient Education: Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
• Clinical Interventions: Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
• Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
• Documentation: Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We’re Looking For

• Licensure: Current and valid Registered Nurse (RN) license in the state of Florida.
• Experience: Minimum of 1 year of experience in a hospital or skilled nursing facility.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.

Compensation Range

$70,379.00 to $105,569.00

The anticipated base salary range represents the Company's good‑faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
