You will perform patient assessments, develop care plans, provide clinical interventions, educate patients and families, and document care in compliance with HIPAA and Medicare standards. Qualifications

• Current and valid Florida RN license.
• Minimum of 1 year of hospital or skilled nursing facility experience.

Responsibilities

• Perform comprehensive patient assessments in home settings, including vital signs, wound care, medication administration, and disease management.
• Develop and implement individualized care plans addressing patient needs and goals.

• Provide direct patient care including wound care, injections, and IV therapy as needed.

What You'll Do

• Patient Assessment: Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
• Care Planning: Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
• Patient Education: Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
• Clinical Interventions: Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
• Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
• Documentation: Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We're Looking For

• Licensure: Current and valid Registered Nurse (RN) license in the state of Florida.
• Experience: Minimum of 1 year of experience in a hospital or skilled nursing facility.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.

Compensation Range

$66,675.00 to $105,569.00
