Job summary

Millennium Physician Group is seeking an RN to join our Home Health Team serving Charlotte County, FL. You will perform patient assessments, develop care plans, educate patients and families, and collaborate with the care team to deliver high-quality in-home care.

Ideal candidates will hold a Florida RN license, have at least 1 year in a hospital or SNF, and bring strong clinical and communication skills with a patient-centered approach.

Qualifications

• Current and valid Florida RN license.
• Minimum of 1 year of experience in a hospital or skilled nursing facility.

How Will You Make An Impact & Requirements

What You'll Do

• Patient Assessment: Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
• Care Planning: Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
• Patient Education: Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
• Clinical Interventions: Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
• Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
• Documentation: Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We're Looking For

• Licensure: Current and valid Registered Nurse (RN) license in the state of Florida.
• Experience: Minimum of 1 year of experience in a hospital or skilled nursing facility.

Compensation Range

$66,675.00

to

$105,569.00

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
