Job summary

Millennium Physician Group is seeking a Registered Nurse (RN) to join our Home Health Team in Lee County, Florida. You will perform patient assessments, plan individualized care, educate families, and provide direct clinical interventions in patients' homes.

Collaborate with the care team to ensure high-quality, compliant care and accurate documentation. The ideal candidate has an active Florida RN license and at least 1 year of hospital or SNF experience, along with strong communication skills

Qualifications

• Florida RN license required.
• Minimum 1 year of hospital or skilled nursing facility experience.
• Strong clinical, communication, and patient-centered care skills.

Responsibilities

• Conduct comprehensive patient assessments including vital signs, medications, wound care and disease management.
• Develop individualized care plans addressing patient needs and goals.
• Educate patients and families on disease management and self-care techniques.
• Provide direct care including wound care, injections, and IV therapy when needed.
• Collaborate with physicians and other professionals for coordinated care.
• Maintain accurate timely documentation of assessments, interventions and progress.
• Adhere to HIPAA and Medicare regulations.

What You'll Do

• Patient Assessment: Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
• Care Planning: Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
• Patient Education: Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
• Clinical Interventions: Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
• Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
• Documentation: Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We're Looking For

• Licensure: Current and valid Registered Nurse (RN) license in the state of Florida.
• Experience: Minimum of 1 year of experience in a hospital or skilled nursing facility.

Compensation Range

$70,379.00 to $105,569.00

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
