Job summary

The CARE Center, Inc. is seeking a full-time Registered Nurse to provide professional nursing and psychotherapy to ACT Team clients in Pennsylvania. You will perform medication checks under physician direction and educate clients and families about psychiatric treatment.

Responsibilities include maintaining charts and documentation, participating in team case conferences, and responding to emergencies as needed. PA Nursing license and psychiatric nursing certification eligibility are required.

Qualifications

• BSN or RN program completion from an accredited school.
• At least 2 years of experience in a psychiatric health care setting.
• Current and valid RN licensure in Pennsylvania.
• ANA certification in Psychiatric or Community Health nursing or eligibility.

Responsibilities

• Provides physician-directed medication checks and education to clients and families.
• Maintains clinical and statistical records; completes re-certifications and forms.
• Assumes lead role in emergency responses when necessary.
• Participates in case conferences with psychiatric and clinical staff.
• Develops individualized treatment plans for ACT/CTT clients and maintains 75% direct service.

Comprehensive Benefits Package • Tuition Discounts • Career AdvancementThe CARE Center, Inc.Full-Time PositionOverall Function:Provides professional nursing and/or psychotherapy to Assertive Community Treatment (ACT) Team services.Essential Functions:Provides physician-directed medication checks to clients and educates consumers and families regarding psychiatric medication treatment services.Maintains close contact with program supervisor and medical director, reporting any reactions to medications, any changes in physical/emotional condition and reporting observations.Maintains appropriate documentation of clinical and statistical records; completes re-certifications and medical information forms and maintains records of medications.Assumes a lead role in responding to emergency situations, when necessary.Serves as a member of the Community Treatment Team or Assertive Community treatment Team as well as the after hours on call rotation.Develops and implements individualized treatment/service plans for CTT/ACT consumers using various therapeutic methodologies, maintaining a direct service level of 75% minimum time.Participates in case conferences with all levels of psychiatric and clinical staff, individually and in supervisor staff meetings.Initiates appropriate Intra and Interagency contact to facilitate consumer care.Prepares, maintains and submits all assigned case records, charts, reports, forms and other required documentation in a professional and timely manner.Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.Bachelor of Science in Nursing or successful completion of a Registered Nurse program from an accredited School of Nursing with two years’ experience in a psychiatric health care setting.Current and valid R.N. licensure to practice in the Commonwealth of Pennsylvania.ANA certification in Psychiatric or Community Health nursing or eligibility for same.Must not be excluded from participation in Medicare, Medicaid or any other federal health care program.Must successfully obtain Criminal History Clearance from PA State Police, Child Abuse Clearance from the PA Department of Human Services and FBI Fingerprint Clearance.Ability to transport oneself and/or clients in an insured vehicle, as program or client needs dictate.Ability to observe client's behavior and keep accurate and detailed charts and records.Available Benefits:Medical and dental benefits for eligible employees.Retirement plan with potential for agency match.Tuition discounts with partnering colleges and universities.Career advancement opportunities.Generous time off for eligible employees.Be an SPHS Hero!EQUAL OPPORTUNITY EMPLOYER
