The CARE Center, Inc. is seeking a full-time Registered Nurse to join the Assertive Community Treatment (ACT) team in Washington, PA. The RN will provide professional nursing and psychotherapy services, focusing on medication management and client education within a psychiatry-focused program.

Eligible candidates will hold a BSN, PA RN licensure, and ANA certification or eligibility. The role emphasizes documentation, case coordination, and emergency response within a collaborative team.

Qualifications

• RN licensure in Pennsylvania and a BSN from an accredited program.
• ANA certification in Psychiatric or Community Health nursing or eligibility.

Responsabilités

• Provides physician-directed medication checks and educates consumers and families about psychiatric medication treatment services.
• Maintains documentation of clinical and statistical records and reports changes in patients.
• Responds to emergencies and participates in ACT/CTT team activities.
• Develops and implements individualized treatment/service plans for ACT consumers.
• Participates in case conferences with psychiatric and clinical staff and coordinates interagency contact.
