Job summary

Birchwood Cottages is a memory care community dedicated to providing compassionate and comprehensive care to individuals with memory impairments. We are currently seeking an experienced Registered Nurse (RN) to join our team as the Clinical Nurse Supervisor for our Memory Care community.

The ideal candidate will be passionate about supporting the health and well-being of seniors, have strong clinical assessment skills, and be able to work collaboratively with both our clinical and non-clinical

Qualifications

• Minimum of 1-2 years of nursing experience, preferably in assisted living, long-term care, or geriatrics.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Must be available to work evenings shifts and the occasional weekend.

Responsibilities

• Conduct assessments of resident health and care needs.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Coordinate and monitor the delivery of nursing services and treatments.
• Act as a liaison between residents, families, and healthcare providers to ensure seamless communication.
• Monitor residents’ conditions, manage medications, and provide medical interventions as needed.
• Ensure compliance with all healthcare regulations and facility policies.
• Complete all necessary reporting and documentation timely.
• Participate in case conferences and provide updates on resident care progress.
• Supervise and mentor nursing staff.
• Provide training and support to new and current staff members as needed.
• Respond to emergencies and provide immediate care when required.
• Understand and have working knowledge of caring for those with memory loss.
• Work in collaboration with the other RN/Clinical Nurse Supervisors and care team.
• Perform other tasks as assigned.

• Must successfully pass a Minnesota background check.
