Hospice Services of Massachusetts seeks a dedicated per diem Hospice RN Case Manager for the North Easton area. You will supervise hospice aides, assess patient and family needs, and provide professional nursing care using the nursing process. Develop care plans and collaborate with the interdisciplinary team to deliver comprehensive care.

Weekly pay up to $50/hour, based on experience. Per diem position with opportunities for career advancement and mileage reimbursement.

Qualifications

• Active MA RN license required.
• Graduate of an accredited school of nursing.
• CPR certification required.
• Hospice experience preferred.

We are seeking a dedicated and compassionate per diem Hospice RN Case Manager to join our team in the North Easton area. As a Hospice RN Case Manager with Hospice Services of Massachusetts, you will be responsible for supervising hospice aides, completing comprehensive assessments of patient and family needs, and providing professional nursing care utilizing all elements of the nursing process. You will develop and implement individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions to patients and families.

Weekly pay up to $50 an hour, depending on experience.
