Job summary

Hospice Services of Massachusetts in Plymouth is seeking a dedicated Hospice RN Case Manager to oversee patient and family needs, supervise hospice aides, and deliver professional nursing care using the nursing process.

You will develop individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions while counseling patients and families to meet nursing needs. Weekly pay up to $50/hr, depending on experience.

Qualifications

• Graduate of an accredited school of nursing.
• Must possess an active and unencumbered RN license in Massachusetts.
• Prior RN hospice experience preferred.
• CPR certified.

We are seeking a dedicated and compassionate Hospice RN Case Manager to join our team in the Plymouth area. As a Hospice RN Case Manager with Hospice Services of Massachusetts, you will be responsible for supervising hospice aides, completing comprehensive assessments of patient and family needs, and providing professional nursing care utilizing all elements of the nursing process. Weekly pay up to $50 an hour, depending on experience.
