Job summary

Hospice Services of Massachusetts is seeking a per diem Hospice RN Case Manager. You will supervise hospice aides, complete comprehensive assessments of patient and family needs, and provide professional nursing care using all elements of the nursing process.

You will develop and implement individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions to patients and families. Prior hospice experience is preferred, but not required.

Qualifications

• Graduate of an accredited nursing program.
• Active and unencumbered RN license in Massachusetts.
• Prior hospice experience preferred.
• CPR certification is required.

Weekly pay up to $50 an hour.

Requirements:
