Job summary

Elara Caring is seeking a Home Health Baylor Registered Nurse in New Haven, CT. This full-time role covers Saturday through Monday 12-hour shifts with a focus on personalized, one-on-one care in the home setting.

You will manage a dedicated caseload with autonomy and the support of an experienced clinical team, ensuring patients recover, manage chronic conditions, and stay independent at home.

Qualifications

• Current, unrestricted RN license.
• Associate Degree in Nursing required; BSN preferred.
• At least one year of clinical nursing experience.
• Home health experience required.

Responsibilities

• Assess patients and develop individualized plans of care.
• Provide skilled nursing services per physician orders.
• Monitor patient conditions, medications, and treatment progress.
• Educate patients and caregivers on treatments and disease management.
• Coordinate care with physicians and interdisciplinary team.
• Maintain accurate and timely clinical documentation.
• Help prevent avoidable hospitalizations and ER visits.

Home Health Baylor Registered Nurse

What You’ll Do

What You’ll Need

• Homecare Homebase experience preferred
• Strong assessment, communication, and care-coordination skills
• Valid driver’s license, dependable transportation, and auto insurance
• Ability to travel throughout the assigned territory
• Ability to sit, stand, bend, lift and move intermittently and lift 50–100 lbs.
