Job summary

Elara Caring in Dallas, TX seeks a Registered Nurse Home Health to join our dedicated team delivering compassionate in-home care. You will assess patients, implement care plans, educate families, and collaborate with the care team to ensure safe, quality service.

The ideal candidate has an active RN license in Texas, at least 1 year of clinical nursing experience, a valid driver’s license, and the ability to travel between patient homes as needed. Home health experience is preferred.

Qualifications

• Current RN license in the state.
• Graduate of an accredited nursing program.
• At least 1 year of clinical nursing experience.

Responsibilities

• Assess patients to determine care needs.
• Complete initial and ongoing patient assessments.
• Provide nursing care per plan of care and orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Maintain HIPAA privacy and confidentiality.
• Assist with transferring patients and equipment as needed.

As a Registered Nurse Home Health ,you’llcontribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
