Job summary

Amedisys is seeking an experienced registered nurse for a per diem home health role in Montgomery, AL. You will coordinate care, educate families, and perform assessments in the patients' homes, tailoring plans with the care team to support healing where patients feel most comfortable.

The position offers a flexible schedule, ongoing professional growth, and the chance to make a meaningful impact through comprehensive home-based nursing care. CPR required.

Qualifications

• Active RN license in the state of Alabama.
• Six months or more home health experience preferred.
• Current CPR certification and ability to manage medications.
• Reliable transportation with valid driver's license and insurance.

Responsibilities

• Coordinate and oversee all aspects of home-based patient care.
• Collaborate with care teams to develop and implement plans.
• Educate families on patient condition, needs and care.
• Perform head-to-toe assessments and monitor vitals.
• Administer injections, IV therapy or tube feedings as needed.
• Complete documentation timely and accurately.
• Supervise LPNs and HHAs and communicate progress to managers.

• Checks vital signs and manages medications.
• Gives injections, IV therapy or tube feedings.
• Administers and maintains catheters.
• Completes documentation timely and accurately.
• Supervises LPNs and HHAs.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.

Qualifications

• One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.*
• Current RN license, specific to the state(s) you are assigned to work.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Able to assess patient status and identify requirements relative to age specific needs.
• Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
• Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
