Job summary

Amedisys is seeking an experienced Registered Nurse to provide in-home patient care across Pennsylvania. You will coordinate care, assess patients, and educate families while collaborating with the care team to implement individualized plans.

Responsibilities include performing physical assessments, monitoring vitals, managing medications, and supervising LPNs and HHAs. A valid PA RN license and CPR certification are required, with reliable transportation essential.

Qualifications

• One year of RN clinical experience.
• Minimum six months home health experience.
• Current CPR certification.
• Current RN license active in the state(s) you are assigned to work.
• Valid driver’s license with reliable transportation.

Responsibilities

• Coordinate and oversee all aspects of patients' home care.
• Collaborate with patient care teams to develop and implement care plans.
• Educate families on patient condition, needs and care.
• Performs head to toe physical assessments and evaluates vitals.
• Checks vital signs and manages medications; administers injections, IV therapy or tube feedings.
• Supervises LPNs and HHAs; documents progress for the clinical team.

• Regularly communicates patient progress to the clinical manager, physician and care team.

Qualifications

• One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.*
• Current RN license, specific to the state(s) you are assigned to work.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Able to assess patient status and identify requirements relative to age specific needs.
• Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

*Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
