Zusammenfassung

Angels Care Home Health is seeking an Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with other medical disciplines and provide professional care, perform admission visits in the home, determine eligibility for home care services, and develop the plan of care to be followed.

Required are a current RN license, a nursing degree from an accredited program, a valid driver’s license, and CPR/Hepatitis clearance.

Qualifikationen

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current state RN license.
• Current state Driver’s License.
• One year’s RN experience (two preferred).
• Proof of CPR and Hepatitis consent/declination.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home & determine the patient’s eligibility for home care services & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
