Job summary

Angels Care Hospice in McCook, Nebraska is seeking a Registered Nurse Case Manager to coordinate all aspects of a patient's home care. You will perform admission visits, assess eligibility for home health services, and develop the plan of care to be followed.

The ideal candidate is a graduate of an accredited nursing program with an active RN license, current CPR, and reliable transportation. One year of RN experience is preferred; drivers license and willingness to travel are required.

Qualifications

• Graduate of an accredited nursing program (diploma/ADN/BSN).
• Current state RN license.
• Current state Driver's License.
• 1 year RN experience (2 preferred).
• CPR certification with hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient's home care.
• Interact with other medical disciplines as needed.
• Perform admission visit to the patient in the home.
• Determine patient eligibility for home care services.
• Develop the plan of care to be followed.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
