Job summary

Angels Care Hospice in Nebraska is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will collaborate with other medical professionals and deliver professional nursing care in the patient’s home, including admission visits and care planning.

Qualifications include RN licensure, CPR, valid driver’s license and 1 year of RN experience (2 preferred). We offer competitive compensation with mileage, on-call pay, generous PTO, training, and benefits.

Qualifications

• Graduate of an accredited nursing program.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year RN experience (two preferred).

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines and provide professional nursing care.
• Conduct admission visits in the patient’s home and determine eligibility for home care.
• Develop and follow the plan of care for each patient.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
