Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate all aspects of patients' home care, ensuring high-quality clinical outcomes. You will work with multiple medical disciplines and perform the patient admission visit in the home to assess eligibility and care planning.

The role requires an active RN license, a nursing degree, a valid driver's license, and reliable transportation. CPR and hepatitis consent/declination are required, with ongoing opportunities for

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current state RN license.
• Current state Driver’s License.
• One year of RN experience preferred; two is preferred.
• CPR certification and Hepatitis vaccination consent/declination.
• Reliable transportation with valid auto liability insurance.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
