Job summary

Angels Care Home Health in Muscatine, IA is hiring a Registered Nurse (RN) Case Manager to coordinate comprehensive in-home patient care. You will collaborate across medical disciplines, perform admission visits, and develop individualized plans of care.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional patient care.
• Perform admission visit to the home and determine eligibility for home care services.
• Develop and follow the plan of care for each patient.

About the position:
