Job summary

Angels Care Home Health in Hutchinson, KS is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate all aspects of patient home care, collaborating with medical teams to deliver high-quality services.

Qualifications

• Must be a graduate of accredited nursing program (diploma, ADN, or BSN) and hold an active RN license.
• Current state Driver's License and reliable transportation.
• CPR certification and Hepatitis consent/declination.
• At least 1 year RN experience; 2 years preferred.

Responsibilities

• Coordinate all aspects of the patient's home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visit and determine eligibility for home care services.
• Develop and follow the individualized plan of care.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home & determine the patient’s eligibility for home care services & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One year's experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
