Job summary

Hopkins Center is seeking a Registered Dietitian to join our team. The RD will apply the nutrition care process to assess residents at admission and during routine reassessments, and will support nutrition-related care across short-term rehab and long-term care units.

You will collaborate with interdisciplinary team to determine plans of care, monitor dietary intake and lab values, provide education to residents and families, participate in care plan meetings and quality improvement initiatives,

Qualifications

• Registered Dietitian credential required.
• Bachelors degree in Dietetics, Nutrition or related field.
• Current state licensure (or eligibility) in New York.
• Previous experience in clinical setting preferred.

Responsibilities

• Utilize the nutrition care process to conduct a comprehensive assessment for residents upon admission and routine reassessment.
• Facilitate nutrition related care for residents on both short-term rehab and long-term care units.
• Collaborate with interdisciplinary care team to determine appropriate plan of care for residents.
• Monitor residents dietary intake, weight, lab values and habits.
• Provide nutrition education and counseling to residents and families.
• Participate in care plan meetings, quality improvement initiatives and nutrition programs.
• Ensure compliance with state, federal and facility regulations.
• Complete MDS assessment in compliance with state guidelines.
• Collaborate with dietary staff for menu planning and to oversee foodservice operations as needed.
