Job summary

Delaware Oaks Center for Rehabilitation is seeking a dietitian to lead nutritional care for residents in a rehabilitation setting. You will work with medical and nursing staff to integrate nutrition plans, update records, and educate residents and families on diet and health.

Preferred candidates hold a BS in Clinical Dietetics and are RD/RDN or CDN certified, with at least one year of experience; training can be provided. Strong interpersonal skills and organizational abilities are essential.

Qualifications

• Bachelor of Science Degree with a major in Clinical Dietetics.
• Preferably Registered Dietitian (RD or RDN) and/or Certified Dietitian of New York (CDN), 1+ year experience (will train the right candidate).
• A member of the Academy of Nutrition & Dietetics.

Responsibilities

• Integrate the food and nutrition aspects of resident care through communication with medical and nursing staff.
• Assist in completing the Minimum Data Set (MDS) as appropriate.
• Contribute information to resident care plans and record assessments, screens and progress notes in EMR.
• Provide nutritional counseling and diet education to residents and families.
• Implement diet changes and modify menus in food service software as needed.
• Support department training and reinforce policies and procedures for dietary staff.
• Attend care plan meetings and develop integrated care plans.
• Observe and advise on food preparation and meal rounds as needed.
• Assist in cost control, staff scheduling, and departmental operations.
• Ensure diet orders are followed and communicate changes to production staff.
