Job summary

Community LIFE seeks a Registered Dietitian to join its interdisciplinary team serving older adults in the Pittsburgh area. You will assess nutritional needs, develop care plans for participants 55+, and provide individual and group counseling to support health and independence at home.

You'll collaborate with the IDT, monitor status, supervise dietary staff, and ensure regulatory compliance at the Day Center.

Qualifications

Bachelor's degree or higher in Nutrition, Dietetics, or related field.
Minimum of one year of experience as a Registered Dietitian with frail older adults.
CDR credential and PA licensure required.

Responsibilities

Conduct comprehensive nutrition assessments and develop individualized care plans for participants age 55+.
Collaborate with the Interdisciplinary Team (IDT) to support participants' overall health and wellness goals.
Provide individual and group nutrition education and counseling.
Monitor nutritional status and make recommendations to improve health outcomes.
Oversee the health, safety, and regulatory compliance of food service operations at the Community LIFE Day Center.
Supervise and support dietary staff.
Conduct in-home visits, as needed, for assessments and ongoing case management (mileage reimbursement provided).
Maintain accurate documentation within the EMR.
