Job summary

PACS is seeking a Registered Dietitian to lead clinical nutrition in a fast‑paced skilled nursing setting in Pittsburgh. You will conduct comprehensive nutrition assessments, develop high‑quality care plans, and collaborate with the care team to optimize resident outcomes.

Responsibilities include coordinating with the Dietary Manager, ensuring accurate MDS entries, and maintaining CMS compliance while delivering safe, evidence‑based nutrition care for aging residents.

Qualifications

• Must be a RD or RDN certified by the CDR.
• Hold a current state license or certification as required by the state.
• 1–2 years of clinical experience in long-term care, sub-acute, or skilled nursing preferred; new graduates considered.
• Proficiency in calculating enteral nutrition regimens and identifying malnutrition.
• Experience with LTC software and executing MDS entries.

Responsibilities

• Complete comprehensive nutrition assessments at admission, quarterly, annually, and during status changes.
• Develop, implement, and monitor interventions for high-risk residents (weight loss, tube feeding, skin integrity, dialysis).
• Participate in weekly interdisciplinary care planning with nursing, SLP, physicians, and PT.
• Ensure timely documentation of nutritional status and interventions in the EHR and MDS.
• Collaborate with the Dietary Manager to review menus and ensure diet plans meet nutritional needs and safety standards.
• Maintain CMS F-Tags readiness and compliance for nutrition/hydration and related practices.
