Job summary

BoldAge PACE seeks a Dietitian to assess nutritional status of participants and implement nutrition interventions within the IDT. You will provide counseling, education, and therapy to participants and families, building care plans and coordinating 24-hour nutrition services.

The role requires a Bachelor’s degree in nutrition/dietetics, passing CDR, and experience with elderly populations; Serve Safe certification is preferred. Join a team dedicated to dignified, home-centered care.

Qualifications

• Bachelor’s degree in nutrition/dietetics or related field.
• Passed CDR exam.
• Minimum 1 year experience with elderly population; if not, training provided on hire.
• Two years clinical nutrition experience preferred.
• Serve Safe certification preferred.

Responsibilities

• Assess nutritional status of PACE participants and plan appropriate interventions.
• Provide nutrition counseling and education to participants and caregivers.
• Contribute to care plan development with the IDT and coordinate 24-hour care delivery.
• Monitor weight, hydration, and dietary intake; report changes to IDT.
• Incorporate cultural preferences into care plans and participate in Quality Improvement initiatives.
